We strongly recommend using
Endurant Faith for this fight, either as an item or as a charm. The majority of Mephisto’s abilities will one-shot the average player build.
Endurant Faith turns those hits into DoTs, avoiding having to deal with one shots, but instead a large DoT that can be healed through or negated through immunity abilities.
Location
You can find Mephisto in Skovos, Atanos; his lair is indicated by a golden dungeon icon seen in the image below.

Phase 1 – Akarat

Sword Beam Sweep – During the opening RP, Mephisto will immediately begin to cast a large swipe with 6 beam frontal. The easiest way to avoid this is to stay at his side while he talks. Alternatively, there is a small gap between the beams you can stand within, but it’s risky.
Dash Stab – Periodically, Mephisto will disappear, then reappear, followed by a lunging attack. Keep moving in wide circles during this, as it targets where you were when he starts the cast, not where you’re going, so no tracking luckily.
Sword Barrage – Mephisto will send out a barrage of red flying swords, again, no tracking on this, so keep moving in a wide circle or use mobility to the side to avoid.
Ground Beam – Mephisto will cast smaller beams similar to the starting ability around him. Red clones will now appear, which will also cast small ground beams. These clones seem to only be cast from the ones closest to him, so stay at range or prepare to leap away if you’re a melee build, as you have a moment to move before the damage comes out.
Sword Rain – In a short intermission, Mephisto will teleport to the middle of the arena and begin floating. Shortly after, the arena will fill with swords, which will begin to swarm towards you. Keep moving in a circle and prepare to use your mobility spells if you’re caught in a bad spot, as they come from random directions.

Red Circles – During the same phase, giant red circles will begin to move around the room, before stopping and exploding shortly after. This is a simple find the safe spot mechanic, or “Don’t stand in the fire”.
Phase 2 – True Form

Raining Fire – As soon as the camera pans out, balls of fire will begin to rain from the sky. Simply keep running in a circle around the room until they stop.
Poison Bombs – Mephisto will stand in the middle of the room. Around Mephisto, and the edges of the room, balls of poison will form before turning into waves. The ones near Mephisto will shoot outwards, and the ones at the edges of the room will come inwards. If these touch you even slightly, the damage will immediately apply, so stand between Mephisto and the far edges of the arena and prepare to dodge.
Orb Rain – Small red clones will begin to form before bursting a barrage of small red orbs. This is another run-in-a-circle mechanic, as they again track where you were and not where you’re going and make sure not to be standing near the red clones as they’re forming.
Death Beam – Mephisto will put both his hands forward and shortly after, begin to channel a large beam ability, and be on the opposite side of him.
Lightning Rain – Periodically, red circles on the ground will begin to form before red lightning will descend. These travel shortly after forming; however, there is a safe spot between them, so try to get behind one and follow it until the mechanic passes. They do slow down and speed up randomly, so make sure you’re not auto-pathing to keep up, or you risk one slowing down and taking burst damage.

Intermission – When Mephisto is at around 25% health, he will teleport to the top of the arena and begin lifting red souls into the air. This is a burst phase, so take advantage and be aware. Directly after it ends, he will send a huge amount of red lines across the arena. These are deadly and tough to avoid. Keep a distance from him once the souls stop flying and get ready to dodge.
Repeats – After the intermission and red lines, Mephisto will continue repeating all the above mechanics until he is dead.
Phase 3 – Nightmare Mephisto

Opening Seconds – As soon as the cinematic ends, Mephisto will immediately cast a massive AoE circle that travels outwards. Stay near the edges of the arena, as it will fade before reaching the far ends.
Lost Abilities – You’ll be without abilities at the start. Around the arena, you will see glowing white orbs; those are your abilities, and you should immediately prioritize getting them back. You do still have access to your Evade during this.
Red Puddles – At the start, there will also be red circles around the arena. Avoid being near these, as they will damage you if touched, but they also send out the red balls from earlier that travel in random directions.
Hand Walls – You’ll see two lines of hand walls randomly. You have a few seconds to get out of these before they collapse into each other.
Fist Smash – Mephisto will smash his hands down in front of him. This will do massive damage if you’re within the area. It will also send outward spirals of red lines from them. Try to get to the side of him and be at a distance to dodge the spirals.
The Heads – You’ll see large heads begin to form around the edges of the room, stand in between a set of these, and shortly after, a massive barrage of red orbs will begin to flow towards Mehpisto.
Omega Beam – Directly after the heads, Mephisto will shoot out a massive, straight line of those red orbs he absorbed. Get behind him and circle behind to avoid getting hit.
Corpses – You’ll see stationary corpses scattered around the arena randomly. Just avoid walking into them, and you’ll be fine, as they are rather small and easily avoided.
Eaten – Mephisto will disappear into the ground, then waves will begin to move inwards towards his location. You will also once again lose your abilities and will need to gather them. Keep moving, and avoid being touched by the waves as they will stun you and suck you inwards. If you’re where Mephisto is pulling you, it is a one-shot, and you will be consumed.
Death Tornado – You’ll see giant red tornadoes slowly moving around the arena; avoid touching these.
Swipe – Mephisto will rear back an arm before sending out a frontal swipe. Stay on the opposite side or behind him.
Red Orbs – You will also periodically see red orbs begin to swarm towards you, similar to the other phases. Keep moving in a wide circle until they run out.
Beat Down – Ending

After all of the chaos, you’re rewarded with a final moment to beat on the Lord of Hatred. He will begin to glow yellow, and it is your opportunity to unleash the frustrations you had during the fight. There are no mechanics during this, and you’ve successfully beaten Mephisto. Congratulations.
